I hereby grant Unity Shoppe permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos will become the property of the Unity Shoppe and will not be returned.
I hereby irrevocably authorize Unity Shoppe to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.

I, volunteer listed below, for ourselves, our heirs, executors and administrators, hereby release, waive, acquit and forever discharge the Unity Shoppe their representatives, successors, insurers, assigns or any other person or entity associated with any of the above organizations such as staff, directors or volunteers, from all liability, claims, demands, or causes of action for any and all loss, damage, injury or death and any claim of damages resulting from use of facilities owned or controlled by the above organizations, or participation in activities of said organizations either at or away from the location. Medical Treatment: I give my permission to the Unity Shoppe to seek emergency medical treatment. I will be responsible for any/all costs of medical attention and treatment.
VOLUNTEER AGREEMENT:
1. All volunteers must sign in and out digital on our Kiosk. We cannot document your hours if you do not sign in and out.
2. Volunteers scheduled for more than 6 hours will need to take a break or lunch.
3. Dress appropriately. No halter tops, crop tops or strapless tops. Wear clean, modest attire with closed toed shoes.
4. No volunteers will be permitted on site if under the influence of alcohol or drugs.
5. Be respectful to staff, clients and other volunteers.
6. No cell phone usage or text messaging will be allowed while volunteering.
7. Unity Shoppe is not responsible for any items mistakenly donated or lost while on the premises. Personal property must remain in your care at all times.
